In 1996, Michael Lubao was processing medical claims by hand; back when claims were still being reviewed on green-bar paper and manually processed line by line. When the National Correct Coding Initiative (NCCI) edits were introduced, he immediately realized that claims auditing and compliance were about to become far more complex. Most organizations saw new rules as a burden; Michael saw an opportunity to build something smarter. He developed the first version of Virtual Examiner® to automate claim review using those NCCI edits—creating one of the earliest systems capable of simultaneously improving payment accuracy and compliance.


Over the next 30 years, PCG never deleted a single rule. Instead, we’ve continuously added and replaced CMS, AMA, and Medicaid updates—building the world’s largest private medical coding directory, with over 72 million edits across 500+ categories.     When President Clinton adopted the NCCI edits in 1996, Michael was running an MSO and doing claims by hand. He knew that he needed to create a coding directory so that it would enable him and his claims team to quickly identify the proper code, modifier, place of service and remain compliant. Each quarter he would update his directory and this directory became the knoweldgebase that over 170 payer organizations have used to pay claims correctly since then.
